Your Admission Information

Your admission

We pride ourselves on providing continued excellence in the health and wellbeing of people in our care. From pre-admission through to discharge, our staff are committed to providing excellent care before, during and after your surgery.

Prior to your surgery we require you to complete some pre-admission forms. This allows us to ensure we have all your relevant details so that we can plan for your stay. You can complete your forms online through our eAdmissions – your secure patient online portal or by hand.

Important information

There is a lot for you to think about prior to coming to hospital for your surgery. We strongly recommend that you read Your journey with us.

You can also take a look at our brochures available here. These all contain important information that you need to know, including what to do the week before, the day before and on the day of your surgery.

Online admissions

Forget the paperwork, complete your pre-admission surgery and procedure forms online via our eAdmissions – your fast, secure and convenient patient portal.

If you would prefer to complete the forms by hand, you can download and print these here. Please ensure you return these to us at least 10 working days prior to your admission.
