Dr Clement Tsang (MBBS, MS, FRACS) is a leading Upper Gastrointestinal Laparoscopic and Bariatric surgeon, surgically trained in Australia. He maintains a primary interest in laparoscopic (keyhole) surgery, with extensive experience in the full range of weight loss procedures. He is conversant in Mandarin and Cantonese, as well as English.

Types of Bariatric Surgery

There are a range of surgeries available, with aspects more suitable for different people of varying circumstances. Dr Tsang offers gastric balloon, endoscopic gastric sleeve, gastric banding, gastric sleeve, gastric bypass and revision weight loss procedures at Waratah Private Hospital.

Intragastric balloon, is a non-surgical weight-loss procedure involving the insertion of a specialised balloon inside the stomach. The balloon serves to occupy space, limiting the capacity available for food to occupy. This increases the satiety of the patient sooner, limiting food intake to smaller portions. The procedure is temporary, and the balloon is usually removed after a period of 6 months (though a 12-monthballoon variation is also available).

This procedure is recommended for those individuals who are not good candidates for weight loss surgery.

We offer two types of gastric balloon, both of which operate in similar ways. These include the Orbera balloon, performed endoscopically under sedation, and the Elipse balloon which is initially swallowed and then filled once it has arrived upon the correct position.

In this procedure, the LAP-BAND® fitted around the uppermost part of the stomach close to where the oesophagus joins the stomach, forming what is called a virtual pouch.

Post-surgery, the diameter of the gastric band can be adjusted to fit the patient’s optimal weight loss requirements and well-being by injecting or removing saline via an injection port placed fixed to the muscle layer.

The stomach is not modified surgically so it is possible to revert back to the normal anatomy at any time by laparoscopically removing the band.The LAP-BAND® System is a clinically proven tool to aid with long-term weight loss. Studies have shown an average excess weight loss of 47.1% maintained at 15 years compared to gastric bypass 54%EWL.

The procedure is performed under general anaesthesia. The surgeon makes small keyhole incisions on the patient’s abdomen. A laparoscope, a thin instrument with a light and camera attached, is inserted through one incision, allowing the surgeon to clearly view the internal organs on a monitor.

Additional surgical instruments are inserted through the other incisions and approximately 80% of the stomach is removed, reducing a 2Litre stomach to 100mls to 150mls size. A thin sleeve shaped like a banana is created by stapling the remaining part of the stomach. At the end of the procedure, the incisions are closed with sutures.

The reduction in stomach size allows the patient to feel fuller faster, while being able to retain the normal functions of the stomach. The method used is simpler than other bariatric surgeries, and allows for relatively less operative time, shorter hospitalization stay and shorter recovery time with minimal postoperative pain and discomfort

The endoscopic sleeve gastroplasty, also known as endoscopic stomach sleeve or ESG, is a nonsurgical weight loss method. The entire procedure is done through the mouth using a camera without any incision.

The ESG is a safe, effective alternative for patients who aren’t ideal candidates for traditional bariatric surgery, or who prefer a nonsurgical weight loss solution.

Typically, patients who undergo this surgery consume 59% lower calories and are able to lose up to 20% of their total body weight two years post-surgery. Due to the non-surgical, minimally invasive nature of the ESG, it has relatively lower risk than other surgical options and the recovery time is relatively quick.

The Gastric Bypass takes advantage of both restriction and malabsorption to work, consequently making it one of the most effective weight loss procedures. Staples are used to create a smaller stomach to limit the amount of food intake, and a large portion of the small intestine is bypassed to limit the nutrients and calories absorbed.

The surgery allows the patient to feel satiety faster, preventing overeating and absorbing lower calories through the modifications made in the small intestines. Additionally, the changes in the anatomy (specifically, the narrowed opening from the pouch to the stoma) means that the food can only leave the stomach slowly, thus providing the patient with prolonged feelings of satiation.

The average expected weight loss with the gastric bypass procedure is around 80% of excess weight in the first year. The weight loss is reduced to approximately 50 to 60%excess weight loss in 10 years
